Please note: All DBA forms are formatted to print on legal size paper. Forms that require the signature of a notary must be completed and notarized before they are brought to our office for recording. As of 9-1-2019 House Bill 3609 requires that all DBA forms dealing with corporations may only be filed at the Secretary of State's office in Austin. We will refuse to file them.

Only Immediate Family Members (either by blood, marriage, or adoption) can obtain Birth Certificates.
All other applicants must provide legal documentation (such as a court order establishing guardianship, an insurance policy listing the applicant as the beneficiary, etc.) that documents a direct, tangible interest in the birth or death certificate.

An Expired Drivers License is not considered a Primary Form of Identification.
When requesting a Vital Record through the Vitals Access Website, you will go through "ID.me", which will verify your Identity.
